Requesting Your Health Care Records
You have the right to request copies of your health care records. To request records from this practice:
- Submit a written request (email or paper form)
- Complete a Release of Information
- Include your full name, date of birth, and specific records requested
- Requests may be subject to applicable fees as allowed by Texas law
- Records will be provided within the timeframes required by state and federal regulations
